Family Educational Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student education records. FERPA regulations require parents (if student is under 18 years old) or eligible students (over 18 years old and/or graduated) to provide
signed and dated written consent, which must be authenticated before disclosing personally identifiable information.
Former Tallmadge High School students who are over the age of 18 and/or graduated may fill out this form to request their own transcript only. If you are a CURRENT THS student, please download or request your transcript through your SchoolLinks account.
Please note:
Third parties (i.e. parents, guardians, etc.) are NOT permitted to request transcripts for students who are over the age of 18 and/or graduated. We cannot provide or order copies of diplomas.
While we make every effort to process your request as soon as possible, the processing time could take up to:
- 3-5 business days during the school year;
- 7-10 business days during summer & holiday breaks
