COVID-19 Vaccination Clinic

Dear Tallmadge Families and Staff,

Akron Children’s Hospital will be offering COVID-19 vaccines for all eligible students, ages 5 and up in the Tallmadge City School District.  In addition to students requiring the 2nd dose, we are also able to accommodate students desiring a 1st dose who may not have been able to participate in the November clinic.

The Tallmadge City Schools Vaccination Clinic is scheduled for:
Vaccine Dose Date:  Monday, December 20
Location:  Tallmadge Middle School, 484 East Avenue, Tallmadge, OH  44278
Time:  3:00pm – 6:00pm

You will again need to register and select a time slot for your child/children through SignUpGenius by clicking here.  Please complete one registration per child.  Time slots are provided in 15-minute increments and are available on a first-come/first-serve basis.  In the SignUpGenius, the first page will have general information and then a list of 15-minute time slots. Click on the time slot you want and then click the Submit and Sign Up button. You will be directed to a second page where you will enter your student’s name, your email address, and your student’s date of birth. When finished, click the “Sign Up Now” button. When signing up, please fill in your student’s name, not the parent’s name in the provided box on the second page. If you have multiple students, please return to the SignUpGenius and complete the entire process for each student. 

If you participated in the 1st dose, you will not need to submit another consent form. If this is your 1st dose through Tallmadge, however, you must complete a consent form for each registered child and return it to your child’s school clinic by Friday, December 17th.  You can download the consent form here.  If you do not have the ability to print from home, you can obtain a copy of the consent form in the main office of your child’s school.

Here is some important information for you to know:
  • Please bring your student’s vaccination card with you to the 2nd dose.
  • Vaccinations are VOLUNTARY.  All health information will be kept strictly confidential by the district and Akron Children’s Hospital.
  • Vaccinations will be administered by Akron Children’s Hospital nursing staff.
  • Students will receive the Pfizer vaccine.  This is a two-dose vaccine.  This will be the second dose of the vaccine. Please review the attached information sheet on the Pfizer vaccine in general from the CDC, here.
  • We have also attached a vaccine information sheet specific to individuals 5 through 11 years of age, here.
  • We ask that parents accompany their child to the vaccine clinic. Please plan accordingly, a 15-minute wait time is required for observation for all students after vaccination.
  • If you believe that transportation might be a hindrance to your child/children receiving the vaccination, please contact, Roy Zeman, Director of Student Services & Data Analytics at [email protected]
  • Physical distancing and COVID safety protocols will be followed, masking encouraged by parents and students attending.
  • Students should be dressed appropriately with an easy access to their shoulder or upper thigh for vaccination. Consider wearing a short sleeve t-shirts

As a reminder, a completed consent form for 1st dose students must be returned to the school clinic by Friday, December 17th
